You also need to REGISTER your PIN NUMBERS to complete the records access process if you have not done so already. You can only collect your PIN NUMBERS from the receptionist at the front desk.
We will now process your request for access to your GP electronic health records and will send you an email to confirm your application has been successful. Please check your emails regularly (including the Junk mail box in case the email is in there). Once you have successfully signed up, we will occasionally send you emails to keep you informed about developments in the practice and also give you the opportunity to learn more about what is going on and for you to share ideas too.
