How to register your family at Thornley House Medical Centre
We welcome new patients living within our catchment area who wish to register with us. Patients who do not reside within our area but who live with a family member who is already a patient at the practice (due to historical boundary changes) may also register. If you would like to register, please check you live within our practice area. You may also like to download a copy of our practice leaflet.
If you would like to register, please contact the Surgery. Or, if you are able, pre-register on-line first. This will help us to process your registration more quickly. Please note that you are not registered with us until you have attended the practice and we have checked your address and documents as per the procedure below.
Once you have pre-registered, please bring the following to the practice and discuss with the receptionist.
- your NHS medical card (if you do not have an NHS medical card, you will need to complete form GMS1)
- proof of your address (eg a utility bill or bank statement no older than three months)
- proof of your identity (eg passport, driving licence or other identification card)
- your child's Red Book if you are registering a child aged six years or under (we will take a copy of immunisation history)
If you are a family registering together, we only need to see the proof of address for one adult family member although you will need to complete form GMS1 (also available from reception) for all family members who do not have a NHS card. If you have recently had a baby, please register the baby's birth and bring with you form FP58 which you will be given at the time of birth registration. You do not need to bring any other documents but it would help us if you are able to pre-register the baby beforehand.
If you are currently registered with another local practice and wish to register with us because you are not happy with your practice, we will be happy to register you but ask that you attend a pre-registration appointment with a doctor to discuss this. Please call us on 0161 367 7910 to arrange this. You may wish to pre-register and bring along the necessary documents ready for registration when you attend for this appointment.
When you attend the practice with your documents, a receptionist will check your details and register you straight away. We like all our new patients to attend for a new patient health check with the nurse and this will be arranged at a time convenient for you when you come to register. We would be grateful if you would bring a completed copy of our new patient health check form to this appointment. -->> document to be added
