WorkWell Project aim –referrals accepted from the 2/9/25

Supporting people who are issued a fit note for mental health related problems or MSK related problems return to work.

Criteria

  • Anyone over the Age of 16 who is issued a fit note for mental health or MSK related issue – discussed in MDT for early, rapid intervention from MSK, Mental Health, Wellbeing practitioner, WorkWell, GP

Data protection impact assessment (DPIA) – submitted to Jane Hill. Awaiting approval

Existing Project – co-produced with input from >700 people in Hyde

Aim: Supporting anyone over the age of 16 – unemployed or struggling at work to return back to work

  • Anyone >16 who is unemployed or struggling in work and could do with further support (WorkWell Coach/Digital Skills training/Comprehensive Health check) – improve clinical coding and gets wrap around support

Patients can be seen at Thornley House Medical Centre on Wednesday

Patients will have a 1 hour slot with a Your Health Matters worker. They do NOT have access to the clinical system but can see them, assess them and start the treatment plan. All you have to do is book the patient into a clinic or you can refer them into the service as below

How to make referral

Make referral using Joy, if not installed this can easily be done via IT. This is the same platform that is used for social prescribing and frailty.

Upon clicking the Joy Widget – You can Make a referral via Your Health Matters”- this should be visible after clicking the plus sign for referral on Joy

If you can add some referral notes –  for e.g. Fit note for MH/MSK related reason, needs support with this, eligible for NHS Health check, poor digital skills or Unemployed, Struggling to find work, poor digital skills, needs support

Upon referral

  • Lauren Danko is our care co-ordinator reviews referral in Joy. Allocated to the correct list and checks if patient is eligible for NHS health check. If eligible then the patient is booked in to have the NHS Health check at the PCN
  • If this is a fit note related referral then patient is booked into the next appropriate weekly MDT so this patient is discussed, and a plan is made.
  • If this is not a fit note related referral patient, then is contacted to have the Your Health Matters Health check and booked into the Monday’s one stop shop. If the patient can’t make this then patient is offered a health check anytime between Mon-Friday (9-5pm) and booked into the next available work well coaches appointment slots.
  • If this is not a fit note related referral, and patient is 16-24 then patient is allocated to Danial who would support this patient and will manage any ongoing issues for them.
