Please remember to also complete a CONSENT FORM if you have not done so for each family member that wants access to their records
You also need to REGISTER your PIN NUMBERS to complete the records access process if you have not done so already. You can collect your PIN NUMBERS from the receptionist at the front desk or ask for this to be EMAILED to you.
We will now process your request for access to your GP electronic health records and will send you an email to confirm your application has been successful. Please check your emails regularly (including the Junk mail box in case the email is in there).
